| What is Linkages?Linkages goal is to help the frail elderly and disabled adults live an independent life of dignity at home by delaying or preventing institutional placement. The Linkages Program provides a caring and qualified staff, including a Case manager, who will meet with you to learn your needs for independent home living. Working together, you'll develop a plan to meet those needs by drawing on community resources. Your Linkages Case Manager will arrange for the assistance you've agreed upon and will check regularly to make sure that the services continue to meet your needs. Should your needs change, your Linkages case manager will alter your plan and its services to meet those needs. Linkages provides information and referral assistance for the elderly and disabled on a variety of long-term care issues. Linkages is not a crisis intervention service for medical, mental health, or other types of emergencies. Nor is Linkages able to provide immediate services prior to enrollment. There are no income criteria for the Linkages program, although clients who can afford to pay are asked to contribute a share of the cost for case management services and/or purchased services. However, no individual will be turned away solely because of inability or unwillingness to pay. The Linkages Program is operated by the Golden Umbrella through a grant from the State of California Department of Aging, the Program Service Area 2 Agency on Aging, local donations and fund raising.
