What is MSSP?

MSSP's goal is to help the frail elderly live an independent life of dignity at home by delaying or preventing institutional placement.

MSSP provides a caring and qualified Case Manager who will meet with you to learn your needs for independent home living.  A Public Health Nurse and a Case manager will provide each client with a complete health and psychosocial assessment to determine the services needed.  The team then works with the client and family to develop an individual care plan  that will draw on existing community resources or services purchased for you.  Services must be provided at a cost lower than that for nursing home placement.

Your MSSP Case Manager will arrange for the assistance that you need and will check regularly to make sure these services continue to meet your needs.  When your circumstances change, you Case Manager will update your plan services to meet your needs.

MSSP is a program operated by the Golden Umbrella through a grant from the State of California Department of Aging and Federal Agencies

These are some of the services that MSSP may help you obtain:

  • Meal preparation or home delivered meals
  • Emergency alert devices
  • Help with governmental or insurance paperwork
  • Physical and occupational therapy
  • Transportation
  • Protective supervision
  • Nursing care
  • Support groups
  • Respite
  • Personal care (bathing, dressing, grooming, etc.)
  • Home repairs and safety equipment
  • legal advice
  • A friendly visitor
  • Psychological counseling
  • House cleaning
  • Shopping
  • Medical monitoring
  • Low-cost housing

To Qualify for MSSP you must:

  • Be 65 years or older
  • Be a Medi-Cal recipient
  • Reside in MSSP Service Area
  • Be at risk of skilled nursing facility placement due to frail health
  • Be in need of additional service or care coordination to live at home
  • Have a willingness to work with a case manager on mutually agreed upon goals

MSSP contracts for the purchase of goods and services from a variety of providers to provide its assistance to seniors.  Please contact the above numbers for more information if you are interested in contracting with the program to be a provider.
